Midwest Advantage HMO SNP (Health Maintenance Organization/Special Needs Plan)
Midwest Advantage is a health plan with a Medicare contract and a contract with the Michigan Medicaid program. It is managed by Midwest Health Plan, Inc., a health maintenance organization, or HMO, based in southeast Michigan. Midwest Health Plan, Inc. contracts with the Federal Government/Medicare to administer your Medicare benefits.
Midwest Advantage is available to anyone who has both Medical Assistance from the State, Medicare (Parts A and B), and lives in Wayne, Oakland, Washtenaw, or Macomb County.
Midwest Advantage provides you with the same coverage you would receive through Medicare Parts A (inpatient hospital care) and B (doctors’ services and outpatient hospital care). Midwest Advantage also includes the Medicare prescription drug coverage (Part D). Beneficiaries will be responsible for a $0 copay for formulary generic drugs and a $0 copay for formulary brand name drugs in the initial coverage stage and catastrophic coverage stage.
