HAP Midwest Health Plan is a wholly owned subsidiary of HAP and a health plan with a Medicare contract and a contract with the Michigan Medicaid program. It is
a health maintenance organization or HMO, based in southeast Michigan. HAP Midwest Health Plan, Inc. contracts with the Federal Government/Medicare to administer your Medicare benefits.
HAP Midwest Health Plan is available to anyone who has both medical assistance from the state, Medicare (Parts A and B), and lives in Wayne, Oakland, Washtenaw or Macomb County
Our plan provides you with the same coverage you would receive through Medicare Parts A (inpatient hospital care) and B (doctors’ services and outpatient hospital care).
It also includes the Medicare prescription drug coverage (Part D). Beneficiaries will be responsible for a $0 co-pay for formulary generic drugs and a $0 co-pay for formulary brand name
drugs in the initial coverage stage and catastrophic coverage stage.
